CSPN TASK PATHWAY
Reflect and adapt a manageable routine
Turn the experience of a week into a realistic action, support request, and review.
For a participant organizing practical routines within guidance already agreed with the care team.
Prepare to begin
Begin with your actual experience of the week. Select one routine that fits your circumstances and keep relevant care-team guidance visible when planning any action.
STEP 1
Observe the week with context
Record daily experience and the support or circumstances around it. Choose one repeated observation and compare an easier day. Keep the observation separate from a possible explanation so the review remains grounded in what happened.
Carry forward: A descriptive pattern and the context that may help you understand it.
Reflect on the seven-day boardSTEP 2
Design a small practical action
Choose a cue, time, place, and action small enough for a difficult day. Identify the support or alternative that could help. Use the care-team guidance already agreed and select the days that fit your current circumstances.
Carry forward: A practical schedule with planned attempts and space to record actual experience.
Rehearse a manageable routineSTEP 3
Adapt from the experience
Describe what you attempted and what happened. Identify helpful conditions and friction, then choose one adjustment to size, timing, setting, or support. Set a review date and preserve the relevant guidance around the routine.
Carry forward: One adjustment grounded in experience and a date to review it.
Review and adapt the planWorked fictional journey
A participant notices that document preparation is harder late in the evening. The reflection board records the context. The routine calendar moves preparation to the appointment-reminder cue and limits the task to one document. The plan review considers whether the change made the next visit easier to prepare for.
Review the completed pathway
- The observation includes timing or context.
- The planned action fits my circumstances and agreed guidance.
- The next adjustment reflects actual experience and has a review date.
