Medication Reconciliation at a GLP-1 Care Transition

A practical medication reconciliation exercise for GLP-1 care transitions and shared responsibility.

The practical question

A care transition may occur after an emergency visit, a specialist consultation, a pharmacy change or a move between practices. AHRQ discusses communication during transitions of care and structured handoffs. In a GLP-1 service, the medication list becomes a shared working document that needs confirmation when responsibility or treatment instructions change.

An original reconciliation exercise starts with the products actually being used. Ask the patient to bring packaging, a current list or securely shared images. Identify the exact name, formulation, prescribing clinician, schedule and most recent use. Compare this information with the record and the receiving clinician instructions. Capture discrepancies as specific items requiring resolution by the appropriate professional.

Original implementation approach

Add medicines obtained from other prescribers, over the counter products and supplements that the patient reports taking. Ask about recently stopped products and instructions issued during the transition. Where a discrepancy affects safe care, route it promptly to a clinician or pharmacist. Keep the final confirmed list accessible through the approved clinical system and give the patient a usable copy.

Assign responsibility for each unresolved item. For example, a pharmacist may confirm the dispensed product while the prescriber clarifies the intended regimen. Explain to the patient what will happen next and who will contact them. The completion step is a confirmed plan that the receiving team and patient can use, with dated documentation showing which instructions were reviewed and who resolved the discrepancies.

Applied learning exercise

Use a fictional record containing a hospital discharge list, an older clinic list and a patient supplied package photograph. Find three discrepancies and write a resolution request for each. Identify the professional who can answer the question and the approved channel for contacting that professional. Then create the final medication summary after the simulation provides the answers. Evaluate how clearly the summary communicates current treatment, recently changed instructions and the timing of the next review.

Use in practice

The reconciliation exercise is an original educational application. Clinical and pharmacy professionals resolve discrepancies within their responsibilities. Use the final list during the next transition and verify that the patient has the same current instructions. The dated resolution record helps the receiving team understand how the plan was confirmed.
