The practical question

A capstone case brings several skills together: clarifying a patient question, finding the relevant source, communicating with the responsible professional and explaining the agreed next step. AHRQ provides SBAR and teach-back tools, while CMS describes prescription drug coverage determinations. An original GLP-1 capstone can combine these domains in a fictional scenario that remains within learner responsibilities.

The case begins with a patient who has a new prescription, an unresolved pharmacy message and uncertainty about the next follow up. Give learners a prescription excerpt, a fictional plan response and a clinic procedure. Ask them to identify the immediate question, the facts already available and the professional who can resolve the remaining issue. The case should make the information sources explicit.

Original implementation approach

The learner produces three deliverables. First, a focused handoff states the question and requested action. Second, a brief tracking entry records the owner and next step. Third, a patient explanation describes the verified plan in usable language. Supply a simulated clinician or pharmacy response before asking the learner to complete the patient explanation. This preserves the distinction between organizing information and making a clinical decision.

Assess the work for accuracy, source use, role awareness, clear communication and completion of follow up. Ask the patient actor to restate the plan. If the response reveals confusion, allow the learner to revise the explanation and repeat the exercise. The debrief should identify one demonstrated strength and one next skill to practice. Supportive revision makes the learning process visible and gives the portfolio a concrete progression point.

Applied learning exercise

Run the capstone in pairs with facilitator supplied responses. Submit the handoff, tracking entry and patient explanation together. Have another pair check every statement against the fictional record and official reference. Then perform teach-back with the patient actor and revise the explanation as needed. Finish with a short reflection naming the source that answered the main question, the professional responsible for the clinical decision and the next learning task suggested by the exercise.

Use in practice

The capstone is an original learning scenario. Local clinical procedures define professional responsibility, and the official references support the communication concepts. Review the final patient explanation against the supplied professional response. The next practice task should address the specific skill revealed by the completed exercise and its teach-back result.
