CSPN TASK PATHWAY

Prepare a focused care conversation

Illustrated pathway for this page

Move from scattered questions and records to a clear agenda and an agreed next step.

For a participant preparing an appointment or a follow-up conversation with a care team.

Prepare to begin

Begin with the appointment purpose, available records, and the care-team instructions about preparation. Keep personal information in your approved private record system.

STEP 1

Organize the information to review

Capture the context, known history, medication list to reconcile, and missing records. Preserve uncertainty explicitly. Use the information confirmation board when you need to track requests and responses.

Carry forward: A list of known information, uncertain details, and records to locate.

Create the discussion inventory

STEP 2

Choose the questions that matter now

Write one question per row, then move the question with the greatest consequence for your next step into first position. Propose the first three as your visit agenda and preserve the remainder for follow up.

Carry forward: A manageable agenda with a clear first question.

Prioritize my visit questions

STEP 3

Capture the agreed plan

After the conversation, record the action owners, timing, receiving contacts, and approved communication route. Read back the plan where possible and record any correction from the care team.

Carry forward: An agreed plan that identifies who does what next.

Record the visit closeout

Worked fictional journey

A participant is uncertain whether the next clinic has received a care summary. The inventory records the summary as requested. The priority board places receipt confirmation first. The closeout records which clinic sends the summary, which contact receives it, and when receipt will be checked.
