CSPN TASK PATHWAY
Learn a concept and explain it clearly
Connect source review, everyday explanation, and a focused clarification question.
For a participant or educator exploring one concept in heart health, cancer care, or survivorship.
Prepare to begin
Choose a concept from an actual educational resource or care conversation. Keep the learning question narrow enough to explore in a short session and record where the concept appeared.
STEP 1
Examine what the resource supports
Record the organization, date, audience, and the statement relevant to your question. Separate what the source explicitly explains from the inference you made. If reviewing two resources, use the comparison desk to keep their audiences and contexts visible.
Carry forward: A traceable explanation and a clearly marked uncertainty.
Review the learning sourceSTEP 2
Build an everyday explanation
Describe what a person does or experiences using familiar words. Connect the term to the question you brought to the conversation. Identify the part that remains difficult to explain and ask the appropriate professional to confirm the meaning.
Carry forward: An everyday explanation linked to a real learning question.
Build a concept bridgeSTEP 3
Practice and refine the explanation
Create a learning card, hide the explanation, and practice saying it in your own words. Reveal the notes to compare. Preserve the clarification question and revisit the card after an educator or professional responds.
Carry forward: A practiced explanation and the question needed to refine it.
Practice with teach-back cardsWorked fictional journey
A participant encounters the phrase care coordination. The source review identifies an explanation about information sharing. The concept bridge asks who sends the next summary. A learning card helps the participant explain the general idea while preserving the individual coordination question for the care team.
Review the completed pathway
- The explanation can be traced to a resource or conversation.
- My everyday explanation uses a concrete example.
- The remaining uncertainty has a specific clarification question.
